How The Autism Care UK Referral Process Works

All enquiries are referred to Maggie Sykes, Operational Manager, Autism and Learning Disability Services. When referring to an initial enquiry detailed requirements should be elicited and the information should establish that the potential Service User meets the basic criteria for placement:

  • Has ASD characteristics and/or a Learning Disability
  • Is over 18 years of age
  • Had identified funding
  • Is likely to be suitable

Once more detailed information is received it will be passed to the Registered Manager of the service deemed most appropriate to meet the needs of the Service User who will assess the reports. In most cases a visit will be arranged by both parties to assess care needs and implications of care provision.

Decision making process

If the decision is made that a placement should be offered, the Operational Manager will inform the referring agency. Once agreed, the proposed care plan will be discussed and detailed costings will be established for the service provision. This detailed costing will then be sent to the purchaser together with a formal offer of placement and an outline contract.

Autism Care UK has a comprehensive transition package which is tailored to the individual. Following on from the referral process, once placement has been agreed a transition planning meeting is held and a transition plan drawn up.

Timeline

A timeline for the transition is then developed which takes account of the needs of the individual Service User, family, current provision and Autism Care. This would include a transition assessment visit to the Service User in their current placement where we gather information for risk assessments, health action plan, pen picture, sensory profile and the AAPEP (Adolescent and Adult Psycho-educational Profile).

This information is then used to develop visual supports, care plans, daily programmes and to plan the transition visits prior to the moving date
